Outreach Advocate

La Esperanza Clinic is looking for an Outreach Specialist to work at our Lakeview clinic at 35 E. 31st St.

The position is during these hours:

     Monday-Friday   8am-5pm

Job Summary

Currently, La Esperanza Clinic reaches seventeen counties of our target population. This position is fulfilled by presentations, health fairs, one on one contact, meetings home visits, clinic visits, businesses, schools, weekend events and other community events as assigned. Flexibility allows the reality of contacts to be achieved. The number of new patient’s users is 1400 over the course of a two year period.

Duties and Responsibilities

The Billing Specialist performs the following duties as assigned.

Functional Responsibilities

  • Facilitate and be the liaison for patients between community and clinic services being sought.

  • Assist in accessing referrals to community resources, including housing, food, clothing, school programs, vocational opportunities or services, and relevant health services.

  • Assist patients in developing natural resources and ensure that appointments are made, being the support network.

  • Coordinate and monitor comprehensive tracking of patients. i.e. Following up by way of telephone call or home visit to ensure new user was able to receive correct services offered by the clinic.

  • Attend and participate in training, staffing, calendar planning, and other agency meetings as appropriate and designated by the supervisor.

  • Attend specialized skill-building trainings.

  • Submit monthly reports.

  • Perform other tasks as assigned by the supervisor.

  • Report to the Outreach Director as approved by the chain of command.

  • Screen patients for the eligibility status in qualifying for assistance programs.

  • Notify the necessary staff members of each patient’s eligibility status.

  • Refer patients to appropriate sources for assistance programs as necessary.

  • Demonstrate flexibility, enthusiasm and willingness to cooperate while working with others in assigned inter-professional care teams to support PCMH programmatic activities.

  • Participate in population management activities, as directed.

  • Assist the care team by helping to measure quality and identify, refine and implement practice improvements that support the medical home, and assist in performance evaluation and quality improvement.

  • Apply the principles of comprehensive, community-based, patient-centered, developmentally appropriate, and culturally and linguistically sensitive care.

  • Answer any questions on the eligibility status of patients.

Specific responsibilities:

  • Identification and Engagement

    • Be the initial contact with the possible old and new patients.

    • Gather data about a patient's entire family so we can direct family members to the right clinic for the service needed. The following will need to be assessed at every new contact:

      1. Completion of an intake.

      2. Information sharing.

      3. Direction and guidance to appropriate provider.

      4. Build up patient volume and follow up.

  • Access Resources and Linking

    • Collaborate and partner with the professional resource network of providers.

    • Partner with and refer to other resources and volunteer organizations in the community for alternatives and support.

  • Service Implementation

    • Direct medical questions to appropriate information by printed data or provider in that medical field.

    • Provide information on the sliding fee, applied discounts, and what to bring to the first visit.

    • Conference with service providers.

    • Maintain a flexible schedule.

    • Effectively communicate with service providers and patients.

    • Co-coordinate and prevent duplication of services.

    • Coordinate activities with Outreach department.

  • Maintain Systematic Records

    • Document all client contacts completely, and submit documentation as appropriate.

    • Maintain comprehensive client files.

    • Typical data to record:

      1. Number of referrals and contacts.

      2. Name and Number of agencies and providers that link services.

      3. Number of presentations to schools and businesses.

      4. Number of patients who receive case management and clinic services.

  • Advocacy

    • Promote and interpret to the patient, community health care partners, and peers, the role of the Esperanza clinic and its services, along with brief case management.

    • Advocate on behalf of La Esperanza Clinic to promote prevention and wellness.

    • Cooperate with the patient and clinic to identify gaps in the system.

    • Participate in social action for program refinement, and ongoing education.

    • Reinforce The Mission, Core, and Value statements

  • Evaluation of Outcomes

    • Work in coordination with the Success by 6 and Outreach Promotor coordinator to plan, process and follow-up to easier achieve patient satisfaction, goal attainment, and efficiency.

    • Decrease fragmentation and duplication in the system.

    • Identify trends and factors contributing to successful service delivery.

    • Improve accessibility to the system.

    • Promote cost containment.

  • Promoting Health

    • Participate in conferences regarding case management.

    • Promote ongoing prevention.

    • Identify opportunities and resources to develop services.

  • Professional Accountability

    • Within the limits of the profession, act to ensure the delivery of quality, cost effective, equitable and timely services in a confidential manner.

    • Provide the appropriate documentation for information gathered and actions taken to promote communication

    • Represent La Esperanza Clinic to internal and external audiences.

    • Promote a positive image.

    • Identify opportunities and initiatives for improving service quality.

  • Maintain professional courtesy at all times.

  • Perform all other related duties as assigned.

Knowledge, Skills and Abilities

Required

  • Experience working in the community.

  • Ability to effectively present information in small group situations to patients, Center staff, and the general public.

  • Excellent interpersonal and written communication skills.

  • Strong organizational skills.

  • Capable of problem solving and working independently.

  • Flexible schedule; must be able to work some evening and weekend hours.

  • Reliable transportation with a valid driver’s license.

  • Knowledge of computer applications including Word, Excel and data base applications.

Preferred

  • Knowledge of clinic and health issues.

Credentials and Experience

Required

  • High School Diploma or GED

  • Customer service experience

  • Bachelor’s degree

  • Experience with Electronic Medical Record

  • Bilingual (English and Spanish) strongly preferred

 

Job Type: Full-time

Pay: $14.00 - $16.00 per hour
